When hiring a new employee, the employee agreement must be filled out completely and sent in. This can be faxed to us to get the person on our system, but the original should be sent to us as soon as possible. The application must be sent or faxed to us the day the person starts to work. When we receive a faxed copy of the application we put an asterisk after the employee's last name. This is done to remind us that this person was set up with a fax copy of the agreement. When we receive the original we take the asterisk off. If the person is a former employee within the last year, they can be rehired with a new Schedule C and a Change of Employee Status Report, by marking the rehire box. If this is done please make sure all the information we have in our system is correct (name, address, social security number, marital status and number of deductions).
